Company Questionnaire

The first section of Digital Onboarding is the “Company Questionnaire” section. Here, you will share information regarding your company, its ownership, physical location, and formation. You will also provide details about your plan.

Navigating to Digital Onboarding

The "Company Questionnaire" includes 6 subsections to fill out. The sections are as follows:

        1. Company Information
        2. Operations Information 
        3. Authorized Signer
        4. Day-to-Day Plan Contact 
        5. Plan / Ownership / Employment 
        6. Retirement Goals and Industry Information

Below, we will give an overview of each subsection. 

Note: As you fill out the information on this survey, it will auto-save, so you can always leave the questionnaire and come back to it if necessary. You will see a green pop-up at the bottom of the screen that says, “Your data has been saved.”

As you complete the survey, you will notice fields marked with an asterisk (*). These are required fields that must be filled out in order for the questionnaire to be submitted successfully

Company Information 

For the “Company Information” section of the questionnaire, you will be prompted to enter general information about your company. This information includes:

  • EIN* - This is your Employer Identification Number and it can be found on your tax documents or origination documents. 
  • Company Legal Name* - This name will be used for official plan documents.
  • Doing Business As (DBA) - Please provide, if applicable.
  • Physical Address* - This is your business’s physical address and should not be a P.O. Box. You must also include the city, state, and zip code. 
  • Company Phone Number* 
  • Company Entity Type* -  This field is a dropdown menu for you to select your business’s entity type, such as “LLC” or another. 
  • State of Incorporation* - The state of incorporation refers to the location where the company was registered. This field is a dropdown menu for you to select the applicable state. 
  • Fiscal Year End* - Your business's fiscal year-end is the date that marks the end of a 12-month accounting period. This field is also a dropdown menu from which you can select the applicable date.

Operations Information 

Below the “Company Information” section is the “Operations Information” section. Here, you will provide information regarding your payroll system and if your company provides email addresses to employees. 

  • What payroll system do you use?* - Select your payroll provider from the dropdown, or select --No provider/In-House--.

  • Does your company provide email addresses and access to computers to their employees?* - Select either Yes or No

Operational Information

Authorized Signer

The next section is the “Authorized Signer” section. Here, you will fill out the information on the authorized signer. The authorized signer is the individual who is authorized to sign legal plan documents on behalf of the business. Please be sure to include the first name, last name, email, and phone number of the individual.

Day-to-Day Plan Contact

Following the “Authorized Signer” section is the “Day-to-Day Plan Contact” section. In this section, you will provide the contact information for the individual who will be taking care of everyday plan duties, whom Vestwell can reach to pass on administrative information or other details. 

If this is the same individual as the “Authorized Signer,” you can easily select the check box next to “Same as Authorized Signer.” If it is a different individual, please fill out their contact information in this section and be sure to include the first name, last name, email, and phone number.

Day-to-day Plan Contact

Plan / Ownership / Employment

The next section is “Plan / Ownership / Employment.” Here, you will provide information about your employees and their levels of ownership. 

Do you currently offer a retirement plan?* - Select either Yes or No.

  • If you select "No," continue to the next step.
  • If you select "Yes," please provide information about your current plan. This will include the plan type, your current provider, and more. You will also be asked to upload your Adoption Agreement or Plan Document and your most recent 5500 filings. Click Upload to submit these documents to the questionnaire.

Do the owner(s) of this company own any other companies?* - Select either Yes or No.

  • If you select "No," continue to the next step.
  • If you select "Yes," please provide information about the other companies. This will include adding the name(s) of the compan(ies) as well as the EIN(s). You may click Add+ to enter additional companies.

Next, you will fill out information on your employees.

  • How many employees (paid via W2) are employed at the company today?* - Fill in the number of employees in this field. 
  • How many employees… - Fill in the number of employees in each below field: 
    • Were paid more than $150,000 last year?* 
    • Are currently an owner of at least 5% of the company?*
    • Are a parent, spouse, child, or grandparent (but not siblings or any other relations of an owner of the company)?*
    • How many employees (paid via W2) do you plan on hiring in the next 12 months?*

Retirement Goals and Industry Information

The final section of this questionnaire is titled “Retirement Goals and Industry Information.” Here, you will select various goals and enter other information about your situation.

  • Please choose your top objective for offering a retirement plan.* - Select one of the four options provided that resonates most closely with your company.
  • Which scenario best describes how you’re thinking about your company contributing to employees' retirement accounts*. - Once again, select one option from the four provided that resonates most closely with your company.
  • Which industry group best describes your company?* - Select one option from the list that best describes your company's industry.

Note: This section will only be available to certain plans. Conversion plans and unbundled startup plans will not see these questions.

Once you have finished all the required fields, you may either select Save to save your submission and return to it later, or Submit
