To request a distribution or roll over assets from a different retirement account, you will need to fill out a rollover form or withdrawal request form in your saver portal.
We require these forms for the following reasons:

- For recordkeeping purposes to obtain proof of employees’ rollover or withdrawal request submissions
- To verify that the request is valid and complies with all plan rules
- For specifications on how and where to allocate the assets
The rollover form provides instructions for savers to share with their prior providers on how they can send the funds to us. The form also has the option to request Letters of Acceptance, which some firms need to initiate a rollover.
